Outreach Dentistry is dedicated to providing Comprehensive qualitycare, quality Geriatric dental care to residents within Long-Term Care facilities throughout the Hamilton-Wentworth region.

A Professional Team
Our team of dentists, hygienists and dental assistants are lead by Dr. James Morreale. Dr. Morreale has a fellowship in Geriatric Dentistry and has been servicing Hamilton's seniors through an outreach program which he developed at St. Peter's Hospital.

We are dedicated to preventing poor oral health among institutionalized seniors. We realize that changes associated with aging present challenges to maintaining an oral health program. To a senior faced with physical limitations, a simple trip to the dentist can become a frustrating, exhausting ordeal.

OUR SERVICE

Without leaving the comfort of your facility, your residents will receive the following services:

	Cleaning		Diagnoses
	Scaling		Treatment plans
	Restorations		Periodontal treatment
	X-rays		Denture cleaning
	Oral examinations		Denture cleaning
	Extractions		Prosthetics
	Mobile Dental Clinic

These services are provided at no cost to your institution! All of the paperwork and administration will be handled by our staff. We will need a room with electricity and water supply as well as cooperation from your staff in marketing the service to your clients through distribution of our newsletters, letters, and brochures supplied by Outreach Dentistry.

Patients will be billed directly.

Your staff will have the opportunity to purchase our booklet and Power Point CD-ROM at a special price for use as an inservice training workshop:

Dentistry & Oral Health for Geriatric Patients

The skill obtained from this workshop will assist the caregivers at your facility in maintaining a high quality oral health program for your residents and meeting the standards of care outlined by the Ontario Ministry of Health.

Mobile Dental services will be provided on a monthly basis with recalls every 6 months. We charge a flat rate for our recalls.
Clients who are interested in our service and recall may choose from the following payment options: They may pay a simple fee for service or join one of our plans

GOLD PLAN

Members of this plan will enjoy the following services:
	2 visits per year	Scaling
	X-ray	Treatment plan & estimate
	Cleaning	Diagnosis

SILVER PLAN

Members of this plan will enjoy the following services:
	1 visit per year	Diagnosis
	X-ray	scaling
	Referral service	Treatment plan & estimate
	Cleaning

SILVER PLAN

Those enrolled in the Silver Plan will enjoy the following services:
	1 visit per year	Diagnosis
	X-ray	scaling
	Referral service	Treatment plan & estimate
	Cleaning

BRONZE PLAN

The Bronze Plan is designed for denture wearers. Its services include:
	1 visit per year	Denture cleaning
	denture identification	Oral examination
	Treatment plan & estimate
